To refer a patient with skin cancer for Mohs surgery, fax the pathology report and the patient's address, phone numbers and insurance information to 404-943-9464. Our office staff will contact the patient to arrange an appointment. Patients will receive a brochure describing Mohs surgery and directions to the nearest office.
